Take Five Rewards Program Announcement & Eligibility

• All Five Seasons Members are invited to become Take Five Rewards Members by providing their email addresses and signing up for the program.
• There are no fees to join.
• No purchase is necessary to join Take Five Rewards.
• Team Members of Five Seasons Family Sports Club and their immediate family members are not eligible for Take Five Rewards.

• You may cancel your Take Five Rewards at any time by contacting the Five Seasons Membership Team.
• Five Seasons Family Sports Club reserves the right to cancel your Take Five Rewards Membership at its sole discretion.

• For Take Five Rewards redemption Club Charge to Account (CTA) balance must be current.
• Take Five Rewards may not be redeemed if CTA balance is unpaid beyond 60 days.
• Points can be earned as soon as the membership application has been processed and activated, usually within 2-4 business days after Five Seasons receives the application. Members can begin earning points as soon as the account is activated.
• Member points automatically expire 3 months after the program ends.
• Points are awarded on net purchases, minus returns and taxes.
• Points and promotional offers may be offered to select members based on purchase activity, program participation or information supplied by the member.
• Points are issued for promotional purposes only when qualifying purchases are made.
• Points have no fixed value, cannot be redeemed for cash and may not be transferred to other individuals.
• Redemption of merchandise, services or gift cards are not eligible for points, except the amount of purchase that exceeds the value of the credit, gift card, or gift certificate.
• Member accounts and points are not transferable and may only be used by an individual member for the member’s personal benefit. Member agrees that the Take Five program and all associated points, rewards, and benefits do not create any property rights in favor of member.
• Member benefits cannot be combined with benefits accrued in a separate account or program.
• Points or gift cards may be subject to tax liability. Any tax liability, including disclosure, is the sole responsibility of the member.
• Five Seasons Family Sports Club shall have no liability for failure, delay or error in crediting points to your Take Five account.

• After earning enough points to claim a gift card, please allow 1 week for receipt/delivery of your Take Five benefit.
• For every reward issued, the appropriate points will automatically be deducted from the member’s account.
• Take Five gift cards may not be used toward payment of Five Seasons Family Sports Club membership account balances.
• Five Seasons Family Sports Club is not responsible for lost or stolen gift cards.

• By enrolling in Take Five Rewards, you agree that Five Seasons Family Sports Club may send you special promotional offers and other mailed or e-mailed communications related to Five Seasons Family Sports Club and its affiliates.
• Members may choose to discontinue mail or e-mail or both communications by contacting the Five Seasons Family Sports Club Membership Team. Members understand that the discontinuation of either form of communication may result in the loss of promotional discounts and offers related to the Take Five Rewards program. Communication via e-mail is a requirement of participation in the Take Five Rewards program.
• Member personal information will be held in accordance with Five Seasons Family Sports Club's Privacy Policy, which can be found in your Membership Agreement.

• Five Season Family Sports Club reserves the right to change the terms, conditions for participation, awards and benefits for Take Five Rewards members at any time without notice and without liability to the members.
• Five Seasons Family Sports Club reserves the right to change or cancel any reward, including increasing the points required to earn rewards.
• Five Seasons Family Sports Club reserves the right to institute blackout dates and restrictions on merchandise, including clearance items, eligible for gift card redemption.
• Fraudulent use of your Take Five Rewards membership may result in cancellation of your Take Five Rewards account and loss of any or all points earned.

• Five Seasons Family Sports Club may discontinue the Take Five Rewards program at any time for any reason without any further obligations to members.
